Police say the accident was reported at about 8 p.m. Monday after an exterior elevator came loose, striking the building and causing debris to fall at Sixth Avenue and West 57th Street.

The Daily News reported that the elevator was hanging from scaffolding 30 stories above the street. The newspaper said the elevator was being used during construction of a residential building.
